LyteMeister walked me through an algorithm asking about acidosis or alkalosis (she had a bicarb of 33 consistent with alkalosis) and hypertension (yes, that's why she came in in the first place).

LyteMeister showed me that her hypokalemia and hypotension was likely related to mineralocorticoid excess and recommended aldosterone and renin level measurement.  Her aldosterone was high and the renin was low. 

LyteMeister stopped giving me advice at this point but I got her an adrenal CT and she had a unilateral adrenal mass.

 

Final diagnosis

Conn's syndrome (solitary adrenal adenoma causing primary hyperaldosteronism causing hypertension and hypokalemia). 

She went for a laparoscopic adrenalectomy.  Months later a follow-up blood pressure was 122/80 and potassium was normal.  

True story!
