What is NOT included?

Most regions have Local Coverage Determinations (LCD's) or Local Medical Review Policies (LMRP's) to supplement the National Coverage Determinations (NCD's).  These LCD's and LMRP's include details for restrictions on additional laboratory tests not included in the NCD's.  These additional tests vary from state to state and sometimes by region within a state are not included in NCDMeister at this time.  Details about which additional tests are restricted for your region are available here from the Centers for Medicare and Medicaid Services website.
